PHYSICIAN CERTIFICATION
INFORMATION
The Marijuana Enforcement Division now REQUIRES medical dispensaries to have a copy of the patient's physician certification.
How to access your Physician’s Certification

Step 1.
Log in to your medical card account on
www.medicalmarijuana.colorado.gov by entering
your username and password. You may have to
reset your password to log in. ​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​
​​​Step 2.
Once you are in your account please click where
it lists your name and card as Active.​​​​​​​​​​​​​​​​

Then Click on the tab towards the top of
the page labeled Provider Certification.​​​​​​​
​​​​​Step 4.
Find the Provider certification that is listed
as Active and click on that one.​​​​​​​​​
​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​Step 5.
Once the Provider Certification loads you
will then click the printer icon in the upper
right hand corner. You can save the file as
a pdf, or in your google files. If saving a pdf
please email it to kikakushmed@gmail.com
